Abstract: 

Background: The Immediate loading (IL) protocol has gained increased popularity among surgeons in the last decade.Objectives: This study aimed to evaluate the clinical performance of microtextured single-tooth implants using IL protocol six months after loading.Patients and Methods: In this case series, SwissPlus implants (Zimmer Dental Inc, Carlsbad CA, USA) which was straight, self-tapping, 12 mm length and 4.1 mm diameter, were inserted into both jaws and single edentulous sites with clinically and radiologically healed bone by an experienced surgeon. In the test group (with IL protocol), one week after the surgery, the provisional crowns and abutments were loaded, and definitive restorations were delivered three months postoperatively. The factors including pain, mobility, probing depth (PD), bleeding on probing (BOP) and marginal bone loss (MBL) were investigated six months after the IL. The Mc Nemar and Wilcoxon tests were used for nominal variables and the paired ttest was also used for quantitative variables.Results: Twenty-five implants were placed in 23 patients at Dental School, Shahed University, Tehran, Iran. The test group included 15 patients (16 implants) treated with immediately loaded single-tooth implants. The survival rate was 100% in both groups. In test, a significant reduction was measured in PD and pain; moreover, no statistically significant difference was noted between follow-ups with regard to mobility and BOP. The mean PD values six months for test and nine months for control after installation were 2.548 mm and 2.694 mm, respectively. Marginal bone loss was significantly decreased in the test group between the follow-ups.Conclusions: Consequently, this study shows promising results for IL of single implants, and it can be considerable that treatment with microtextured surface implants is a predictable and appropriate method of treatment.

Abstract: 

Background. This systematic review aimed to determine the effectiveness and outcomes of immediate loading methods for short dental implants. Methods. The authors independently conducted an electronic search in the PubMed, Embase, EBSCO, ProQuest, and Cochrane databases for relevant articles published until November 15, 2020. The references of the included studies were assessed, and a manual search was conducted in Google Scholar and PubMed to find additional relevant studies. Results. Finally, three studies were selected and included in this systematic review. Significant heterogeneity existed in the design of the included studies, and due to the low number of the included studies, the authors could not perform a meta-analysis. The studies showed that the survival rate of immediate-loaded short implants is comparable to conventional loading methods. However, more marginal bone loss is expected. Overall, the immediate loading of short dental implants might be clinically successful. Conclusion. Based on the results, immediate loading protocols might be safely used for short implants. However, caution should be exercised in interpreting these results. Future welldesigned randomized clinical trials with more participants and study power are necessary to support the findings of this systematic review.

Abstract: 

Background & Aim: The absolute advantages of immediate loading of dental implants, such as time saving, immediate function and patient satisfaction, increases the demand for this protocol. In this article we report the results of literature review on conditions under which necessary for successful immediate loading and discuss the predictable conditions as well.Materials & Methods: Search of relevant published articles was conducted for the period between 1970 to 2007 in Medline, PubMed, Embase, Blackwell Synergy, Sciencedirect, Scholar google, Google and Yahoo databases along with manual search to identify pertinent English text-books. The key words used for this search were dental implants, immediate loading, early loading, delayed loading, occlusal loading and primary stability. Factors that may influence the success of immediate implant loading, including patient selection, type of bone quality, required implant length, micro- and macrostructure of the implant, surgical skills, the need for achieving primary stability/control of occlusal forces, and prosthesis guidelines, were thoroughly reviewed and discussed.Results: Immediate loading is predictable in the anterior section of the mandible, irrespective of implant type and surface topography (success rate 90% - 100%). Limited evidence is available for edentulous maxilla and partially edentulous patients. Unfortunately most of the reviewed articles provided limited information due to study design and methodological shortcomings.Conclusion: Anatomical locations implant designs, and restricted prosthetic guidelines are a key to ensure successful outcomes. Future studies, preferably should be more of randomized control trials and prospective longitudinal studies are certainly needed before this approach can be widely used.

پس از بازدید کارشناسان از Loading Arm و بررسی شرایط محیطی و انجام تحقیقات بر روی Loading Arm های مشابه، کاتالوگ ها و مرجع های معتبر گروه در قالب استانداردهای مرجع مهندسی و بدون کپی برداری کورکورانه شروع به طراحی کامل نمود که با توجه به داده های موجود و شرایط کارکرد منجر به طراحی تمام مکانیزم ها گردید. طراحی و مدل سازی Loading Arm در قالب نرم افزار Solid Works انجام شد تا بتوان تمام مکانیزم های حرکتی این طرح را به طور کامل شبیه سازی3D نمود و مورد تحلیل قرار داد. طراحی کامل همراه با جزئیات کامل برای ساخت در داخل همراه با شبیه سازی و تحلیل دقیق مکانیزم های حرکتی ازجمله Haydrulic Jac, Swivel Joint و ... در زمان بسیار کوتاه از نتایج این طرح است.

Abstract: 

Introduction: The concept of treating edentulous patients by osseointegrated implants was first proposed in 1960s. To minimize the failure rate of implants, it has been recommended to keep the implants free of load during the healing period .Recent studies have been directed to achieve faster integration and shorter healing periods prior to implant restoration. Loading protocols (immediate loading , early loading) can best be interpreted on the biologic basis of implant integration. Each of these protocols needs special prerequisites and there are special risk factors for different loading protocols. Objectives: In this review article, different loading protocols and their surgical and prosthetic considerations are discussed.

Abstract: 

Background and Aim: It is believed that a period of healing, approximately 3-4 months for mandible and 4-6 months for maxilla, are required to ensure osseointegration. Recently, a new method called Immediate loading of implants have been proposed. This method provides improved stability of the implant by the immediate support of an iterim prosthesis. The aim of this study was to evaluate the success rate of the Immediate loading single OPILS dental implants.Materials and Methods: Thirty-nine patients, including 26 females and 13 males, at the minimum age of 18 and maximum age of 54 years old, received 48 OPILS dental implants. In this prospective study, all samples were non-smoking, without any systemic disease or psychological problem. In all cases, and acrylic prefabricated crown was placed immediately after implant surgery. The definitive metal-ceramic crown was placed 2.5 months later in mandible and 5.5 months later in maxillae. Lack of pain, pus, implant immobility and less than 1mm bone loss were criteria of success.Results: The oral hygiene of females were better than males, 26 women had plaque index (0) and 11 of them had plaque index (1) where as out of 13 male subjects, 4 had plaque index (0), 6 had plaque index (2) and (3) had plaque index (3). The mean vertical bone loss, on mesial and distal parts, were less than 1mm, with the min. of 0 and max. of 0.4mm and the average of 0.2mm. All implants pass the defined criteria of success.Conclusion: On the basis of this short-term study, in single tooth cases, OPILS implants of Neo-Dent system can be loaded immediately.

Abstract: 

Background and Aim: considering the limitations of conventional loading time and reported failures of immediate load protocol and on one hand, report of an article starting the success of Sargon expandable implant system, the aim of this study was to evaluate the short-term success rate of immediately loaded expandable implants placed in fresh extraction sockets.Materials and Methods: This quasi clinical trial study included 24 Sargon implants placed in fresh extraction sockets in 20 patients, aged from 23 to 70 years (average: 14.79 ± 13.99 years) which were immediately provisionalized. The final restorations were delivered at 3rd week follow up if the implants were stable. Data were collected at 1, 2, 3 weeks and 1, 2, 3 and 6 months. Sex, region, bruxism and clenching and smoking status were studied as related factors. The failure rate and influence of the related factors were analyzed using Fisher's exact test.Results: Due to leak of patent compliance, 2 of the 24 implants were excluded from the study. The load was removed off of 10 implants (45.4%) that were unstable after immediate loading, and the expansion mechanisms were reactivated for those cases. All the cases became stable later on and they were reloaded. Lower bone quality was statistically associated with a nine fold increase in risk of loosing stability (p<0.001). After a 6-month follow-up period, a success rate of 100% was reported for all implants.Conclusion: The results indicate that immediate loading of expandable implants in fresh extraction socket can be a predictable procedure. A true clinical trial study to evaluate the efficacy of this implant over a longer follow up period is necessary.
